PWD ID Registration
Requirements:
- Medical Certificate specifying the type and cause of disability
- Barangay Certificate or Certificate of Residency
- Birth Certificate or valid government ID
- 1x1 or 2x2 ID photo (recent)
- For renewal: Expired PWD ID
Process:
- Obtain medical certificate from physician
- Visit the PDAO Office at the Municipal Hall
- Submit all required documents
- Fill out the registration form
- Have your photo taken
- Wait for ID processing (3-5 working days)
